This article explains key areas to be considered when reviewing patients with mental health problems. It introduces a template which is useful guide for generating a care plan.

Why is it important?

Mental health review dine properly can really improve patient experience and help patient and their families to have clear idea of their care plan. This will enable clinicians to do a holistic review of patients and cover key areas such as social aspect affecting mental health, static and dynamic factors in one's life contributing towards poor mental health.

Perspectives

Whilst writing this article we learnt a lot, especially the importance of exploring psychosocial aspect of patients when reviewing their medication. we realised that having a standardised template to act like a checklist for clinicians doing a mental health review will make reviews meaningful and promote good medical practice. Patients will receive a care plan which will be easy to understand and it will sign post pathways for them depending on their needs.
