The Road to Home is comprised of the teaching tool, the documentation used by nursing staff, the Cookies & Counts classes for additional teaching opportunities and family/nurse satisfaction with the discharge process for newly diagnosed oncology patients

Why is it important?

To enable families to learn in a timely fashion, everything needed to take their newly diagnosed oncology patient home for the first time after diagnosis. It allows for discharge teaching to start shortly after admission and diagnosis so that everything is not left until the day before discharge
