Why is it important?

The ease of use of LigaSure, the reliable and comfortable harvesting of skeletonised or pedicled ITA with the reliable hemostasis (without cauterisation or hemostatic clips), and the speed up of harvesting as no need to change the instruments may lead to more widespread use of BITA. Similarly, Maryland jaw LigaSure device and specially designed retractor systems for LITA harvesting through thoracotomy may ease the MIDCAB procedure.

Perspectives

When writing this article, I thought I could be evenly misunderstood and criticized for advertising a device. On the other hand, I decided that I should write, considering how skeptical and conservative as cardiac surgeons approaching innovations. I have written despite all the criticism it may take, believing that this device, which has been used successfully and safely for almost 2 decades by many surgical disciplines, can be used with precision and confidence to harvest ITA. And it will increase the use of ITA and BITA to the high level it should be.
