What is it about?
• The HOME Program supports patients after they discharge from the psychiatric inpatient unit, offering telephone-based outreach and a home visit until the patient is engaged in care. • HOME Program contacts include suicide risk assessment, safety planning, and problem-solving around barriers to care. • As compared to a control group, participants who enrolled in the HOME Program were more likely to engage in care, engaged in care more quickly and attended more outpatient appointments. • Treatment engagement is a well-established protective factor against suicide.
