Long has been discussed about several factors that may be compromising Root Cause Analysis from accomplishing its goal, to prevent adverse events to happen again. In this article we discuss findings from several studies in this manner.

Perspectives

I hope this article contributes to the understanding of the other causes around Root Cause Analysis effectiveness. If we address adverse events as a whole we can improve patient safety and avoidable adverse events could be prevented.
