What is it about?

One year follow-up of 4 eyes treated with intravitreal aflibercept (Eylea) injection for complications of retinal artery macroaneurysms (RAM). This study showed that a single injection of aflibercept in the vitreous cavity might lead to resolution of macular swelling and improve vision. Closure of RAM was noted in each eye with no recurrence over one year of follow up.

Why is it important?

This study is the first report in the literature of intravitreal aflibercept injection in the management of complications of RAM. Unlike most other published reports on RAM, all patients involved in the study were followed for at least one year. Findings at 3,6 and 12 months were reported for the sake of uniformity. Successful closure of aneurysm was observed in each eye with no recurrence over one year period of follow up. Intravitreal aflibercept (Eylea) should be considered for the management of complications of RAM.

Perspectives

The study showed that patients with macular edema caused be leakage from RAM can successfully be treated with intravitreal aflibercept injection. One patient had previously failed avastin. However, anti-VEGF injections might not be appropriate for thick subfoveal hemorrhage from RAM as subfoveal blood results in scar formation and poor vision despite successful closure of RAM.
