What is it about?
the current case report highlights the presence of self-mutilation in a case of paranoid schizophrenia. although self-harm is quite common in the context of substance abuse, borderline personality, Munchausen disorder, etc it is less commonly reported in schizophrenia. we considered various differential of the case including cannabis-induced psychosis, organic psychosis, non-convulsive epilepsy, an obsessive-compulsive disorder, etc, but after a thorough history, serial mental status examination, appropriate scales, supporting investigations and presence of first-rank symptoms the provisional diagnosis of schizophrenia was considered. no previous history of self-mutilation was present in the patient. family history for schizophrenia was positive in mother while the brother had a depressive episode. the premorbid personality of the patient was not suggestive of any abnormal traits
