What is it about?

Older people are “falling through the gaps” and experiencing broken up care. This is especially true for older people living with more than one long-term health problem and physical limitations. To solve this problem, it is important to come up with answers that reflect and respect the voices of older patients. This review aimed to find and summarise the research on older patients' experiences and journeys through the health system. We will use the findings of this research and work with older people to design local initiatives to improve care for patients.
