What is it about?

Hypoxaemia in patients prior to general anaesthetic always requires very careful assessment. We report a very unusual case of right-to-left cardiac shunting which was caused and treated by alterations in thoracic anatomy

Why is it important?

Our case highlights the importance of considering a right to left cardiac shunt in the differential of those with hypoxaemia. It also illustrates the importance of ordering appropriate investigations, such as a bubble study, in the work up of those with hypoxaemia. Furthermore the case exhibits the potential influence of thoracic anatomy on oxygenation.
