Why is it important?

Results suggest that pharmacist involvement in discharge counseling and follow-up may be important in improving medication adherence and reducing re-hospitalizations. Furthermore, results from this study suggest that pharmacists providing discharge counseling can be feasible. We believe that data obtained from this pilot study identified important information that should be made available to other investigators working on the relationship between medication adherence and 30-day readmission rates.
