What is it about?

Sysmex XN analyzers supply six lymphocyte positional parameters: LY-X, LY-Y, LY-Z, LY-WX, LY-WY and LY-WZ. We collected 301 samples from 301 patients with different lymphoproliferative disordes and mononucleosis syndrome and compared the values of the six parameters.

Why is it important?

They are useful to differentiate expansions of T large granular lymphocytes from other lymphoproliferative disorders and to separate mononucleosis syndrome cases from other diagnostic groups. In 91 cases (19 without lymphocytosis) the analyzer did not show any morphologic flag and in 66 cases (13 without lymphocytosis) the lymphocyte positional parameters showed some abnormal values . This was very important for the HCL group since in 6 out of 7 total samples the analyzer did not generate any morphologic flag but the lymphocyte positional parameters were abnormal in 5 cases.

Perspectives

We proposed new algorithms using the lymphocyte positional parameters provided by Sysmex XN analyzers that are useful to differentiate expansions of T large granular lymphocytes from other lymphoproliferative disorders and to separate mononucleosis syndrome cases from other diagnostic groups. Finally these parameters are very useful to detect changes in the lymphocytes of samples without morphologic flags that make it necessary to review a blood smear.
