What is it about?

This state of the art paper resumes the latest consideration about the analysis of alveolar-capillary diffusing capacity (DLCO) and its constitutive components (membrane conductance Dm and capillary blood volume Vc). The famous Roughton and Forster equation (1957) has been revised. Impact of some physiological terms on the outcome, including ambient oxygen pressure, mean intracapillary oxygen partial pressure, Hb concentration and specific gas conductance (θCO and θNO) has been evaluated

Why is it important?

By using real and simulated data, the authors showed that: 1. Double-diffusion (TLNO-DLCO) would be considered as the only reliable method for studying Dm and Vc. 2. The values of DmCO and Vc depend critically on the chosen θ value. 3. The calculation should base on a finite value of reaction rate between NO and Hb (θNO). The interpretation of new Dm and Vc values might bring different information

Perspectives

Based upon those statements, conventional algorithm for estimating Dm and Vc by should be reconsidered.
