What is it about?
Clinical risk management aims to identify, analyze, and avoid errors and risks systematically in order to improve patient safety. In our study we analyzed near misses detected by team timeout in 18 081 ophthalmic patients. 53 cases of “wrong side” and 52 cases of “wrong intraocular lens” near misses were noted.

Why is it important?
Team time-out proved valuable, as it improved the patients’ safety with minimum effort. Errors may occur despite several preoperative controls and can be detected by performing team time-out.
