What is it about?
Health care is essential for persons living with long-term health conditions. In this paper, we identify what are the barriers to access care by income group in 20 countries.

Why is it important?
We study the case of people with spinal cord injury (SCI). SCI combines a high level of physical impairment with a series of chronic illness, characteristic that makes this group one of the most vulnerables: high-needs and high-costs. Thus, SCI could serve as an index case to analyze how the health system respond to people facing disability.
Perspectives
Demographic dynamics suggest that disability will soon be at the center of population health. Hopefully, this article will show the readers two things: One, the need for the health system to adapt to better respond to people facing disability; and two, that there is more than one reality in every country. Inequalities show that barriers to access health care are quite different for the poor compared to the rich. We need more targeted measures.
