What is it about?

This publication is about the use of a group method of care delivery for patients with multiple long term conditions. It provides the evidence of a patient led intervention to improve self-management and patient empowerment.

Why is it important?

It challenges the way in Community Care in how we develop patient led services that focus on what is important to the person. It also focuses on the benefit and strength that group learning brings to those lining with multiple long term conditions and how they learn from each other.

Perspectives

This research challenges the way we interact and plan our interventions of care with those living with long term conditions. The research enable a co productive stance with regard to the development of the intervention. In turn this improved self management outcomes and self efficacy in their care.
