Matching coordination to complexity: integrated care for older adults with multiple needs

What is it about?

Older people with multiple physical, cognitive and social needs often receive fragmented care. We show that light-touch coordination offers little benefit, while intensive, multidomain and technology-supported care is more effective when matched to the complexity of individual needs
