Why is it important?
Transitional care for children with chronic neurodevelopmental disorders such as ADHD is an area of national crisis and no validated guideline is available to inform practice in UK. This is an example of best practice to be shared among professionals.

This article provides a local initiative and an example of good practice that other professionals working in diverse regions across the UK may find useful as a source of inspiration or useful lessons.
