What is it about?

The Canadian population is aging, and one way to provide better care for older adults is through services coordination and partnerships with different care organizations. A research that studied the factors that support successful services coordination occurs between different health and social care organizations occurred for three Canadian older adult programs situated in Alberta, Ontario and Nova Scotia.
