What is it about?

Propafenone, a well known class Ic antiarrhythmic agent, also shows 1.) the typical chemical structure of a beta-blocker, 2.) an increase of plasma concentrations during exercise, i.e., a typical property of beta-blockers, and 3.) single oral doses of 600mg propafenone show clinically effective beta-blockade

Therefore, we believe that propafenone should be regarded both as a class Ic AND a class II antiarrhythmic agent

Perspectives

Thus, single doses of 600mg propafenone appear suitable for cardioversion of paroxysmal atrial fibrillation even in patients with structural heart disease since 600mg propafenone is a beta-blocker by itself and herewith explicitly indicated according to the recent ESC guidelines in patients with coronary artery disease (particularly following myocardial infarction) and heart failure
