Why is it important?
The struggle to provide meaningful and successful services to persons with mental illnesses id getting worse. Budget cuts mean we need tho think about delivering and following p on care for those most vulnerable populations who need our assistance.
Perspectives
The term Care Coordination has begun to get more use int eh lexicon of health care services. Our program has been using the term since the late 1990's to describe the inter-relatedness of all of those who touch our lives every day - even those beyond the traditional "healthcare" industry. This sense of collaboration is essential in a "whole person" approach to planning and the actual implementation of identified services and shows amazing results.
