What is it about?

Intraventricular hemorrhage is one of the most grave morbidity among preterm neonates in NICU. This morbidity complicates condition of neonates and evokes serious concerns about the possible further neurological sequels. Although several studies have been done to investigate neonatal risk factors. only A FEW studies have focused on maternal conditions . In this study, we recruited all maternal morbidities and especial conditions and evaluated the possible association between these conditions and occurrence of IVH in their neonates.

Why is it important?

This study introduces a new concept in evaluating IVH in neonates and highlights role of antepartum and even preconception maternal health status in certain neonatal suffering. Some of the findings of this study parallels with those of some previously done researchers and some of the findings are completely novel achievements.

Perspectives

The study supports the probable association between IVF and IVH. This finding has been reported in a very few studies and in some of these studies, this association has been attributed to confounding variables like weight and age of the neonates born after IVF as a result of more frequent multiple gestation in this group. However, this study supports an independent association between IVF and IVH in infants after controlling for all probable confounding variables. This finding could be a base for basic science studies, to evaluate the possible underlying pathology like the increased vasoactive receptors on endothelial surface of neonatal brain vasculature or the decreased protection of the scaffolding soft tissue that surrounds the vessels , which can be the result of quantitative decrease in intercellular matrix or change in the structure of the scaffolding compositions in neonates born after IVF. Another noticeable finding of this study was an increased rate of IVH in neonates born to mothers, who have undergone laparoscopic surgery because of infertility. This association has been reported for the first time in this study, and further investigations seem to be necessary to clarify the issue and seek the possible underlying pathologies or the possible confounding variables that have been ignored in this study. An association between prolonged rupture of membrane and occurrence of IVH in preterm infants, is another proof for the possible role of infection and/ or inflammation in rendering the neonates to IVH.
