What is it about?

We evaluated the central venous catheter-related bloodstream infection patient safety indicator, a metric devised by the Agency for Healthcare Research and Quality (AHRQ). We compared it to the National Healthcare Safety Network (NHSN) central line associated bloodstream infection (CLABSI) metric and found that they produce significantly different results.

Why is it important?

The NHSN and AHRQ metrics have different definitions and consequently produce different results. We do not recommend replacing the NSHN CLABSI metric with the AHQR PSI metric to measure healthcare associated bloodstream infections due to central venous catheters.

Perspectives

We found that the AHRQ PSI has a very low sensitivity for detecting CLABSI cases, producing an infection rate that is less than one-third that determined via NHSN methodology. Administrative coding for this healthcare associated infection is not a good tool for widespread use as a surveillance method.
