Perspectives

This is a simple educational image-based case showing the importance of chest X-ray and echocardiogram interpretation for asessment of positioning of cardiac implantable electronic devices, such as a ICD, as is shown in the presented case. In addition, it is also mentioned a simple maneuver to abolish ICD firing, i.e. the placing of a magnet over the generator. In fact, the magnet inhibits device sensing and thus tachyarrythmia detection and in this way avoids ICD overdrive pacing and shock (tachyarrythmia treatment). Having a magnet in all EDs is mandatory to solve cases like this.
