What is it about?
This case report presents a 46-year old man with a failed liver transplant who developed respiratory distress. He was diagnosed with pulmonary blastomycosis and started therapy with liposomal amphotericin. In spite of therapy, he clinically worsened, developed acute respiratory distress syndrome (ARDS) and eventually expired.

This case report provides an overview of a severe case of pulmonary blastomycosis and provides a brief overview on laboratory diagnosis and the current recommended therapeutic guidelines to treat severe blastomycosis.
Perspectives
Pulmonary blastomycosis is a systemic pyogranulomatous infection that arises after infectious conidia of the fungus Blastomyces dermatitidis are inhaled. Infections can be very severe in immunocompromised individuals, and suspicion of this organism requires prompt clinical evaluation and management to reduce morbidity and mortality.
