What is it about?

High-sensitivity cardiac troponins have improved the diagnostic process for patients with acute chest pain, but there are conflicting data about whether to use a single cutoff value for both sexes or sex-specific cutoff values as in females.

Why is it important?

We analyzed evidence for high sensitivity cardiac troponin cutoff values at admission in patients of both sexes who suffered from acute chest pain. High-sensitivity troponin values under the Limit of Detection (LoD), or between the LoD and the 99th URL, would help to quickly rule out myocardial infarction at emergency department presentation, independent from sex specific cutoff values.

Perspectives

Almost all studies have focused on ruling-out myocardial infarction early, and values of high sensitivity cardiac troponin within the LoD allow to do so with a reliable certainty (Negative Predictive Value >99.5%). Conversely, to rule-in myocardial infarction, at least two measurements are necessary.
