What is it about?

This case report describes a rare condition in which blood clots formed spontaneously inside dilated veins of a left-sided varicocele in a man with diabetes mellitus. The patient presented with acute scrotal and inguinal pain without preceding trauma, surgery, heavy exercise, or known coagulation disorder. Color Doppler ultrasonography demonstrated: Grade III left varicocele Venous reflux Echogenic intraluminal thrombi within dilated scrotal veins The report emphasizes the role of ultrasound in establishing the diagnosis and excluding more serious causes such as secondary varicocele or tumor-related venous thrombosis.

Why is it important?

Spontaneous thrombosis of varicocele veins is extremely uncommon and may be overlooked clinically. The condition can mimic other causes of acute scrotal pain, including epididymitis, incarcerated hernia, or testicular torsion. Early Doppler ultrasound recognition can prevent unnecessary surgical exploration or misdiagnosis. The case highlights the need to distinguish: primary versus secondary varicocele benign thrombus versus tumor thrombus Diabetes-associated endothelial dysfunction may contribute to venous thrombosis even in the absence of recognized coagulation disorders. The report adds to the limited literature regarding spontaneous thrombosed varicoceles in adults.

Perspectives

Larger studies are needed to clarify the true incidence and risk factors of spontaneous varicocele thrombosis. The possible relationship between diabetes mellitus, endothelial dysfunction, and venous thrombosis in varicoceles warrants further investigation. Future research may evaluate whether thrombosed varicoceles represent an underrecognized cause of acute scrotal pain. Standardized ultrasound diagnostic criteria could improve recognition and differentiation from secondary malignant venous thrombosis. Additional studies comparing conservative and surgical management strategies may help guide optimal treatment recommendations.
