What is it about?

A serious facial injury can threaten much more than appearance—it can also put a person's sight at risk. Fractures involving the middle part of the face are often accompanied by injuries to the eye, optic nerve, or surrounding tissues. Unfortunately, these vision-threatening problems are sometimes missed because medical teams must first focus on stabilizing life-threatening injuries. Even a short delay in recognizing certain eye conditions can lead to permanent and irreversible vision loss. This review brings together the best available evidence on the most serious eye complications associated with midface trauma, including orbital compartment syndrome, traumatic optic neuropathy, open-globe injury, eye muscle entrapment, retinal injury, and bleeding inside the eye. Based on this evidence, we developed a practical step-by-step framework to help emergency physicians, maxillofacial surgeons, ophthalmologists, radiologists, and trauma teams recognize these injuries earlier and manage them more effectively. The framework emphasizes early vision assessment, recognition of warning signs, appropriate imaging, timely specialist referral, and coordinated multidisciplinary care. It also highlights that some traditional treatments, such as routine high-dose corticosteroids for traumatic optic neuropathy, are not supported by current evidence for every patient. Our goal is to help healthcare teams reduce preventable blindness, improve patient safety, and deliver more consistent, evidence-based care for people with facial trauma. Although the framework should be validated in future clinical studies, it offers a practical approach that hospitals can adapt to improve visual outcomes worldwide.

Why is it important?

Vision-threatening eye injuries after midface trauma remain a major clinical challenge because they can be overlooked during the initial management of patients with severe facial injuries. In many cases, preventable vision loss results not from a lack of available treatments, but from delayed recognition, inconsistent assessment, or poor communication between specialties. This review is important because it integrates current evidence into a practical, systems-based framework that supports timely diagnosis, standardized clinical assessment, and coordinated multidisciplinary care. Rather than focusing on a single injury, it addresses the entire spectrum of serious ocular complications and provides clinicians with a structured pathway for decision-making in emergency settings. By promoting earlier recognition of high-risk conditions and evidence-based management, this framework has the potential to improve patient safety, reduce avoidable blindness, and enhance the quality and consistency of trauma care across different healthcare systems.

Perspectives

As an ophthalmic surgeon involved in the management of patients with facial trauma, I have seen how visual outcomes often depend not only on the severity of the injury but also on how quickly eye complications are recognized and how effectively different specialists work together. This experience motivated me to develop a practical framework that bridges emergency medicine, maxillofacial surgery, radiology, and ophthalmology. I hope this work encourages clinicians to adopt a more systematic approach to ocular assessment after midface trauma and stimulates future prospective studies to validate and refine the proposed framework. Ultimately, my goal is to contribute to safer trauma care, reduce preventable vision loss, and improve the quality of life of patients worldwide through better multidisciplinary collaboration and evidence-based clinical practice.
