This case report is about diagnosis of umbilical cord entanglement. In this case, it was caused by a monochorionic diamniotic twin pregnancy with spontaneous septostomy of the dividing membranes.

Why is it important?
This is the first report of umbilical cord entanglement diagnosed using dual-gate Doppler imaging. Umbilical cord entanglement is the leading cause of fetal mortality in monoamniotic twin pregnancies. No method provides an absolute diagnosis in published literature.
Perspectives
It is very difficult to provide an absolute diagnosis of umbilical cord entanglement, but very important for prenatal management. Dual-gate Doppler imaging enables the recording of two different heart rates at two different segments within the lesion of entanglement, depicted by color flow mapping. This method is easier and more accurate than conventional methods.
