What is it about?
The Background Acute pancreatitis is an inflammation of the pancreas that is frequently caused by gallstones blocking the bile or pancreatic ducts. To prevent recurrent and potentially deadly complications, the standard medical treatment is to remove the patient's gallbladder. Current medical guidelines recommend performing this surgery during the patient's initial hospital stay. During recent periods of extreme strain on the healthcare system, such as the COVID-19 pandemic and regional military conflicts, doctors were not always able to perform these surgeries immediately. The conductors of the current study wanted to find out if delaying the surgery led to worse health outcomes for patients suffering from mild gallstone-induced pancreatitis. Researchers reviewed the medical records of 141 patients who were treated at Soroka University Medical Center between 2017 and 2024. They compared the outcomes of two groups: • Patients who received early surgery within 10 days of diagnosis. • Patients who received delayed surgery after that time frame. The Findings - The study revealed that delaying the procedure did not harm the patients: • There were no significant differences between the two groups regarding complications, readmission rates, or the number of days spent in the hospital. • Factors such as the patient's age, gender, and whether the surgery was delayed had no impact on the likelihood of a prolonged post-operative hospital stay. • The only factor that significantly impacted recovery time was the actual duration of the operation itself.

Why is it important?
The Takeaway message: While we still advocate following standard guidelines for early surgery when possible, the results are reassuring. If a hospital is overwhelmed due to crises like a pandemic or war, delaying gallbladder removal for patients with mild biliary pancreatitis is a safe alternative that does not cause dire consequences.
