In this study, we propose an autologous fat grafting method that includes deep ligament release injections and superficial blunt cannula layering techniques, which together can achieve high patient satisfaction among people with structural dark circles (SDC).

Why is it important?
Infraorbital dark circles have become a troubling issue for many young people, giving a tired and aged appearance. A significant portion of these dark circles is caused by structural deformities in the lower eyelid area. Many articles have shared their experiences using autologous fat to treat structural dark circles (SDC). However, some studies focus on deep-volume augmentation of the lower eyelid, neglecting the nutritional support for the superficial skin, while others do the opposite, emphasize the superficial layer and overlooking the correction of deep tear trough deformities. We believe that to ensure a good response, it is essential to treat both the deep and superficial layers of the lower eyelid.
Perspectives
The autologous fat grafting technique presented in this study is a relatively safe and effective method for treating SDC. An obvious improvement in dark circles and tear trough deformity were observed. Participants overall reported high satisfaction with the aesthetic outcomes. Furthermore, our fat injection technique can be combined with other treatment methods, such as lower lid blepharoplasty or midface augmentation, allowing for personalized treatment based on the specific characteristics of each patient.
