What is it about?
Alignment and position of total knee prosthesis is crucial for outcome. For a long time the gold standard was neutral alignment with regards to mechanical axis. However, there is an increasing debate about how to align total knee arthroplasty in each patient. This study showed that neutral mechanical alignment should be used in patients with no bow shaped legs as it leads to better outcome after total knee arthroplasty.

Why is it important?
The study is the first one using 3D imaging to assess postoperative alignment. 3D-CT is highly accurate and hence one can be certain that the alignment data obtained perfectly reflect reality.
Perspectives
This study forms another puzzle part on the way to a more personalised alignment of total knee arthroplasty. It shows which patients might better undergo mechanical neutral alignment and in which the results are inferior with such alignment method. In future, the alignment in total knee arthroplasty will be further personalised.
