The 2026 World Conference on Lung Cancer (WCLC 2026) featured several important advances in small-cell lung cancer (SCLC), including developments in treatment strategies, disease monitoring, and emerging therapies for relapsed disease.
The phase 3 MAVERICK (SWOG S1827) trial investigated approaches to monitoring for brain metastases in patients with SCLC following initial treatment. The findings showed that MRI surveillance alone improved cognitive outcomes compared with MRI plus prophylactic cranial irradiation (PCI), with no apparent difference in in the preliminary analysis.
Two phase 3 studies, TAISHAN-302 and ARTEMIS-008, evaluated B7-H3–targeted antibody–drug conjugates (ADCs) against topotecan in SCLC. Both ADCs improved survival compared with topotecan supporting their potential as emerging treatment options in this setting. However, differences between the enrolled populations and the fact that both studies were conducted exclusively in China may restrict cross-trial comparisons and limit the global applicability of the findings.
DeLLphi-308, an early-phase study, evaluated subcutaneous tarlatamab in patients with previously treated extensive-stage SCLC. The study provided early insights into the safety and antitumor activity of this potentially more convenient method of tarlatamab administration. A phase 3 trial, DeLLphi-315, is ongoing to compare the efficacy and safety of subcutaneous and intravenous tarlatamab.
SCLC remains one of the most aggressive forms of lung cancer, with limited treatment options and historically poor outcomes. The data presented at WCLC 2026 highlight ongoing efforts to improve care across multiple aspects of disease management, from treatment delivery and monitoring to the development of new therapies for relapsed disease.
Several studies evaluated a range of approaches, including subcutaneous administration of tarlatamab, MRI surveillance alone as an alternative to MRI plus prophylactic cranial irradiation, and B7-H3-targeted antibody-drug conjugates for relapsed SCLC. Together, the findings improve understanding of how different aspects of care may be optimized and identify potential future treatment options.
These findings reflect continued progress in addressing unmet needs in SCLC and support ongoing research aimed at improving outcomes and quality of life for patients.
